Characteristics of Inflammatory Markers with ARDS Events in Covid-19 Patients Confirmed the Degree of Severe and Critical at H Adam Malik Hospital Medan

Introduction

The Covid-19 pandemic, caused by the SARS-COV-2 virus, has led to a significant increase in cases of acute respiratory distress syndrome (ARDS) worldwide. ARDS is a life-threatening condition that can occur in patients with Covid-19, especially those with severe and critical conditions. This article discusses the clinical characteristics and inflammatory markers in Covid-19 patients who experience ARDS, with a focus on data obtained from the H Adam Malik Hospital in Medan, Indonesia, during the period of February to July 2021.

Research Methods

This study is a retrospective analysis of 204 medical records of patients who were confirmed positive for Covid-19 with severe and critical degrees. The data was taken from patients treated at H Adam Malik Hospital Medan. The inflammatory marker laboratory was analyzed to compare groups of patients with ARDS and non-ARDS groups.

Research Result

Of the total 204 patients, the average age of the patient was 57 years, with 119 of them (58.3%) being male. More than half the number of patients, namely 116 patients (56.9%), experienced ARDS. Among the ARDS groups, 58 patients (28.3%) were included in the elderly category (≥ 60 years). Nearly half of patients in the ARDS group (42.6%) had at least one chronic comorbidatory disease.

Compared to non-ARDS groups, ARDS patients showed a higher prevalence for hypertension (34.8% vs. 22.1%). In addition, they had a higher number of neutrophils (11.07 vs 7.57 x 10³/μl), which shows neutrophilia (43.6% vs 13.2%), and significant lymphocytopenia (47.1% vs 9.8%). The neutrophil to lymphocyte (NLR) ratio in ARDS patients was also higher (13.87 vs 10.31), with an increase in procalsitonin (43.1% vs 13.7%).

Discussion

The results showed that old age had a close relationship with an increased risk of ARDS and death. This may be caused by an increasingly weak immune response as we get older. Inflammatory markers such as the number of neutrophils, NLR, and procalsitonin can function as an important indicator in identifying COVID-19 patients who are at high risk of experiencing ARDS.
