Introduction

Postoperative pain is a significant concern in patient care after undergoing surgical procedures, including cataract surgery. It is a universal experience that can affect anyone regardless of their background, age, or social status. The quality of life of patients can be significantly impacted by inadequate pain treatment, leading to disrupted sleep patterns and affecting mental health. In this context, anesthesia used during surgery plays a crucial role in determining the level of pain felt by the patient. This article aims to evaluate the comparison of postoperative pain in patients who use local anesthesia compared to general anesthesia at the Sumatra Medical Eye Center (SMEC) Hospital in Medan.

The Importance of Anesthesia in Pain Management

Anesthesia is divided into two main categories: local anesthesia and general anesthesia. Each type of anesthesia has a different postoperative pain effect and level. Local anesthesia numbs a specific area of the body, while general anesthesia induces a state of unconsciousness. The choice of anesthesia depends on the type of surgery, the patient's medical history, and the surgeon's preference. In this study, we aim to evaluate the effectiveness of local anesthesia compared to general anesthesia in managing postoperative pain in patients undergoing cataract surgery.

Research Methods

This research is an analytical descriptive study with a cross-sectional design conducted at the Sumatra Medical Eye Center (SMEC) Hospital in Medan. Data collection was carried out from September to November 2016 using the total sampling technique. Primary data was collected using the Visual Analogue Scale (VAS), which measures the level of pain felt by patients after cataract surgery. The assessment was done by asking the patient how much pain they experienced in the first 8 hours after surgery, using a numerical scale of 1-10.

Research Results

The results showed that of 34 patients who used general anesthesia, the majority felt that there was no pain, with 30 people (88.2%) reporting no pain. While 3 people (8.8%) experienced mild pain, and 1 person (2.9%) felt moderate pain, no patients experienced severe pain. On the other hand, of 115 patients who used local anesthesia, the most complaints were mild pain, experienced by 59 people (51.3%), while 54 people (47%) felt painless. Only 2 people (1.7%) experienced moderate pain, and no patients reported severe pain. Statistical analysis using the Fisher test showed significant results with a correlation coefficient (p = 0.001), which shows a significant difference in the level of pain between the two groups.

Conclusion

Based on the results of the study, there are significant differences in the level of postoperative pain between cataracts between the use of local anesthesia and general anesthesia at the Sumatra Medical Eye Center (SMEC) Hospital in Medan. Patients who used general anesthesia tended to experience a lower level of pain compared to patients who used local anesthesia. These findings can be an important guide in clinical practice to choose anesthesia that is more suitable for patients in managing postoperative pain. Given the impact of pain on the quality of life, the selection of appropriate anesthesia must be considered carefully by the medical team.

Additionally, the study only measured postoperative pain in the first 8 hours after surgery, and did not assess pain at later time points. Future studies should aim to recruit larger sample sizes and assess pain at multiple time points to provide a more comprehensive understanding of postoperative pain.
