Acute myocardial infarction ST elevation (IMAEST) is a serious medical condition that requires immediate attention and effective management. The onset of symptoms plays a crucial role in determining the treatment outcome, and the goal of revascularization is to restore blood flow to the affected heart muscle and improve tissue perfusion. One indicator of the success of blood flow restoration is the value of Timi Flow 3 accompanied by a good Timi Frame Count (TFC). This study aims to investigate the connection between ST segment elevation and Timi Frame Count after revascularization in IMAEST patients with onset under 12 hours at H Adam Malik Hospital Medan.
Acute myocardial infarction ST elevation (IMAEST) is characterized by typical symptoms, increased cardiac enzymes, and electrocardiogram (ECG) images that show the ST segment elevation. Effective management from Imeest is very dependent on the onset of symptoms. The treatment aims to restore blood flow to the affected heart muscle and improve tissue perfusion. One indicator of the success of blood flow restoration is the value of Timi Flow 3 accompanied by a good Timi Frame Count (TFC).
This study was conducted from January 2019 to August 2020 and involved 60 patients with Imeest who had onset of symptoms under 12 hours and underwent primary cardiac catheterization (IKP) intervention procedures. ECG examination 12 leads were carried out before and 90 minutes after the revascularization, where the sum of the ST segment elevation in the related leads was assessed. The success of the revascularization process was evaluated using the Timi Frame Count.
Of the total 60 patients studied, the results showed that 30 patients (50%) had a bad timi frame count, while 30 other patients had a good timi frame count. Further analysis revealed several variables related to poor TFC, including Killip class, Timi risk score, onset of symptoms, systolic blood pressure, serum ureum levels, troponin I level, ST segment elevation before revascularization, ST elevation 90 minutes after revascularization, as well as the percentage of ST resolution. Among all the measured variables, significant correlation values are found in the Paska revascularization ST elevation (R 0.710, P Value 0.001), changes in ST elevation (ΔST) (R -0.767, P Value 0.001), and ST resolution (R -0.827, P Value 0.001).
The conclusion that can be drawn from this study is that the sum of the ST segment elevation is closely related to the poor TFC value. Especially, there is a strong correlation between Ste 90 minutes after the revascularization and the ST segment resolution with poor TFC conditions. This shows that the monitoring of the ST segment elevation can provide important information in determining the success of revascularization in Imeest patients. This discovery is beneficial in clinical practice, as a basis for making better decisions in patient management with Imeest, as well as to improve the overall prognosis and clinical results.
In this context, it is important for medical personnel to carefully supervise the ST segment elevation and adapt the handling strategy in accordance with the patient's response after intervention. Further research is needed to understand the mechanism behind this relationship and to explore new methods in increasing the results of revascularization in patients with Imeest.
