The Influence of Demographics and Knowledge of Commercial Sex Workers on HIV/AIDS on the Use of VCT Clinical Services in Toba Samosir Regency

Introduction

Human Immunodeficiency Virus/Acquired Immune Deficiency Syndrome (HIV/AIDS) is a significant public health concern worldwide, including in Indonesia. In Toba Samosir Regency, North Sumatra Province, the number of people living with HIV/AIDS is ranked second highest in the province, with a total of 61 cases reported until 2007. Commercial sex workers (CSWs) are one of the most vulnerable groups at risk of contracting HIV/AIDS due to their occupation. This study aims to investigate the effects of demographics and knowledge of CSWs about HIV/AIDS on the use of Voluntary Counseling and Testing (VCT) clinical services in Toba Samosir Regency.

Background

HIV/AIDS is a chronic and life-threatening disease that affects the immune system, making individuals more susceptible to infections and diseases. The virus is primarily transmitted through unprotected sex, sharing needles, and from mother to child during pregnancy, childbirth, or breastfeeding. In Indonesia, HIV/AIDS is a significant public health concern, with a growing number of cases reported annually. The North Sumatra Province is one of the provinces with the highest number of HIV/AIDS cases, with Toba Samosir Regency ranking second highest in the province.

Methodology

This study employed a survey approach using the Explanatory Research method to collect data from 55 CSWs in Toba Samosir Regency. The survey aimed to gather information on the demographics and knowledge of CSWs about HIV/AIDS. Data was collected through interviews using a questionnaire and analyzed using multiple linear regression. The study aimed to identify the factors that affect the use of VCT clinical services among CSWs.

Results

The results of the analysis show that the variables that affect the use of the KPAD Balige VCT Clinic include age, length of work, income, and knowledge related to HIV/AIDS. However, the educational variable was found to have no significant effect on the use of VCT clinical services. Among the variables, knowledge about HIV/AIDS was found to be the most dominant factor that affects CSW access to VCT clinical services. Specifically, knowledge about the high risk of HIV/AIDS transmission was found to have the greatest impact on their decision to take advantage of these services.

Discussion

The findings of this study highlight the importance of increasing awareness and understanding of CSWs about HIV/AIDS to reduce the number of new infections among them. The study suggests that knowledge about HIV/AIDS is a critical factor that affects CSW access to VCT clinical services. This is consistent with previous studies that have found that knowledge about HIV/AIDS is a key determinant of behavior change among CSWs.

Recommendations

Based on the findings of this study, the following recommendations are made for the Toba Samosir District Health Office:

Form a community for people with HIV/AIDS (PLWHA): The district health office should establish a community for PLWHA to provide support and services to individuals living with HIV/AIDS.
Take a more intensive approach to CSWs: The district health office should take a more intensive approach to CSWs, including the delivery of comprehensive information about HIV/AIDS and the benefits of using VCT clinical services.
Increase awareness and understanding of HIV/AIDS: The district health office should increase awareness and understanding of HIV/AIDS among CSWs through education and outreach programs.
Provide access to VCT clinical services: The district health office should ensure that CSWs have access to VCT clinical services, including free or low-cost testing and counseling.

Conclusion

This study highlights the importance of demographics and knowledge of CSWs about HIV/AIDS on the use of VCT clinical services in Toba Samosir Regency. The findings of this study suggest that knowledge about HIV/AIDS is a critical factor that affects CSW access to VCT clinical services. The recommendations made in this study aim to increase awareness and understanding of HIV/AIDS among CSWs and provide access to VCT clinical services. By implementing these recommendations, the district health office can reduce the number of new infections among CSWs and provide protection to the most vulnerable groups.
