Introduction

Chronic Obstruction Pulmonary Disease (COPD) is a serious health problem that often goes ignored. It is a leading cause of morbidity and mortality worldwide, affecting millions of people. In Indonesia, COPD is a significant public health concern, particularly in urban areas like Medan. This study aims to describe the profile of COPD sufferers who are stable in three major hospitals in Medan, namely the Adam Malik Hajj Center General Hospital, Deli Tobacco General Hospital, and Pirngadi General Hospital, during 2010.

Background

COPD is a progressive lung disease characterized by airflow limitation, which is not fully reversible. It is often caused by long-term exposure to lung irritants, such as tobacco smoke, air pollution, and dust. The disease can lead to significant morbidity and mortality, particularly among the elderly and those with a history of smoking. In Indonesia, COPD is a major public health concern, with a significant burden on the healthcare system.

Research Methods

The method used in this study is descriptive, where researchers collect and analyze data from the medical records of patients diagnosed with COPD. The retrospective approach provides insight into the patient's condition based on existing data, without any intervention from researchers. The data was collected from patients undergoing examination at the polyclinic of the three hospitals during the period January to December 2010.

Research Results

The results showed that the highest total COPD sufferers were found in Pirngadi Hospital with a total of 411 people. The majority of patients are over 60 years old, reaching 63.7%. Most patients are retirees, with a percentage of 44.9%. In addition, the Batak tribe dominated the population of patients with a percentage of 62.3%, and the majority of patients settled in the Medan region, which was 76.6%.

In terms of financing, most patients use Health Insurance (ASKES) with a percentage of 85.2%. Interestingly, more than half of patients (50.9%) are former smokers, and based on the Brinkman index, the degree of the most COPD severity is in the weight category, which is 73.6%. The overall COPD degree category also shows a weight number, with a percentage of 49.1%. The most widely used drugs by patients are the Xanthin group (36.1%), while for inhalation drugs, a combination of long-acting beta agonist (profit) and steroids dominate with a percentage of 57.1%.

Discussion

The results of this study provide a clear picture of the distribution of COPD sufferers based on sociodemography, degree of disease, smoking status, financing sources, and treatment in the three major hospitals in Medan. The findings show that the majority of patients are over 60 years old, retired, and from the Batak tribe. The high prevalence of COPD among the elderly and former smokers highlights the importance of awareness and prevention efforts.

The results also show that most patients use Health Insurance (ASKES) as their financing source, which is a positive trend. However, the high prevalence of former smokers among COPD sufferers is a concern, as smoking is a major risk factor for the disease. The use of Xanthin group drugs and a combination of long-acting beta agonist (profit) and steroids for inhalation therapy is also notable.

Conclusion

From the results of this study, it can be concluded that there is a clear picture of the distribution of COPD sufferers based on sociodemography, degree of disease, smoking status, financing sources, and treatment in the three major hospitals in Medan. This finding shows the importance of awareness of COPD, especially among the elderly and former smokers' populations, as well as the need for special attention to appropriate aspects of treatment to improve the quality of life of sufferers. By understanding this profile, it is hoped that efforts to prevent and manage a better disease in the future can be made.

Based on the findings of this study, the following recommendations are made:
