Introduction

The implantation of permanent pacemakers is a common medical procedure that is performed to improve heart rhythm disorders. Every year, more than 500,000 implantation of permanent pacemakers is carried out throughout the world. The pacemaker serves to send electrical impulses to the heart muscle to improve heart rhythm disorders, which can occur due to interference in the formation or delivery of heart electric impulses. Assessment of the quality of life of patients after the implantation of permanent pacemakers is very important to predict the effectiveness and success of the therapy. In addition, this assessment can also be used to evaluate clinical factors and cost effectiveness of this health technology.

Background

The quality of life of patients after implantation of the pacemaker is a crucial aspect of postoperative patient management. The assessment of quality of life can be used to evaluate the effectiveness of the therapy and to identify factors that affect the quality of life of patients. In this study, we aimed to assess the quality of life of patients after implantation of the pacemaker using the SF-36 questionnaire.

Methodology

This study was conducted at the Cardiology Department of H. Adam Malik Hospital Medan from January 2021 to July 2023. A total of 60 outpatients who had undergone an implantation of a permanent pacemaker with a minimum duration of 6 months were involved in this study. The patients were assessed using the SF-36 questionnaire, which is a widely used instrument to measure the quality of life of patients.

Results

The results of this study showed that there were 60 patients involved in this study, with the majority of patients being men (56.7%) and their average age was 67.98 ± 10.95 years. Most patients (81.7%) have comorbidities, which is a significant factor that affects the quality of life of patients. As many as 93.3% of patients have one-space pacemaker, of which 68.3% of the ventricular cable is located in the non-apical segment of the heart.

The assessment of quality of life reveals that there is a relationship between age, gender, and location of the pacemaker with almost all domains on SF-36 (p <0.05). In addition, there is a significant relationship between comorbidities and SF-36 scores, which include physical function, restrictions on the role due to physical health problems, and restrictions on the role due to emotional health problems (p <0.05). Significant relationships are also found between the ejection fraction and SF-36 scores, including physical function, restrictions on the role due to physical health problems, vitality, social function, body pain, and general health. However, no relationship was found between the body's mass index and the duration of the Implantation of the pacemaker with a SF-36 score (P> 0.05).

Conclusion

Thus, this study shows that monitoring the quality of life after the implantation of the pacemaker must be an integral part of postoperative patient management.
