Introduction

Tuberculosis (TB) is a major public health concern in Indonesia, with the country implementing a TB eradication program through the application of DOTS (Directly Observed Treatment Short-course) strategies. The Kotanopan Health Center, located in the Kotanopan District, has been working to control the spread of TB, but the number of cases has shown fluctuations, and the healing rate of patients with pulmonary TB has not reached the national target of 85%. This study aims to analyze the management of the Lung TB Management Program with a DOTS strategy at the Kotanopan Health Center.

Background

TB is an infection caused by the bacterium Mycobacterium tuberculosis. In Indonesia, the TB eradication program is implemented in stages in the Puskesmas through the application of DOTS strategies. The DOTS strategy involves the use of a combination of drugs to treat TB, with the goal of curing the disease in the shortest possible time. The Kotanopan Health Center has been implementing the DOTS strategy since 2014, but the number of cases of suspected TB pulmonary has shown fluctuations. In 2014, there were 261 people suspected of having TB, which decreased to 247 people in 2015, and increased again to 280 people in 2016.

Research Purposes

The main purpose of this study was to analyze the management of the Lung TB Management Program with a DOTS strategy at the Kotanopan Health Center. The method used in this study was a descriptive survey with a qualitative approach. Researchers conducted in-depth interviews with informants chosen purposively, in order to be able to understand clearly and in-depth about the management of the pulmonary TB prevention program.

Research Findings

Based on the results of the study, the discovery of the pulmonary TB case in the Kotanopan Health Center was carried out through a field visit. The diagnosis of pulmonary TB follows the DOTS procedure, namely by microscopic sputum examination. Treatment of pulmonary TB is given in the form of Fixed Dose Combination (FDC), which aims to facilitate patients in taking drugs. In addition, supervisors to take medication (PMO) are tasked with monitoring and reminding patients in undergoing treatment. The supply of anti-tuberculosis drugs (OAT) is also always available.

Recording and reporting is carried out in accordance with the format specified in the TB report. Monitoring and evaluation is carried out in conjunction with supervision to the Puskesmas to ensure the implementation of the program runs according to plan.

Analysis

Although the Kotanopan Health Center has carried out various efforts in dealing with pulmonary TB with DOTS strategies, the success of this program is still low. The healing rate that has not reached 85% shows a significant challenge in program management. Several factors that can affect the low healing rate include:

Weak inter-sector coordination: Cross-sectoral and cross-program cooperation needs to be strengthened in order to support each other in TB control.
Level of public awareness: Socialization of the importance of TB treatment and discipline in taking drugs needs to be improved so that people better understand the impact of this disease.
Limited resources: The Kotanopan Health Center may not have sufficient resources, including funding, personnel, and equipment, to effectively implement the DOTS strategy.
Inadequate training: Health workers may not have received adequate training on the DOTS strategy, which can lead to poor implementation and low healing rates.

Recommendation

To increase the effectiveness of the pulmonary TB control program at the Kotanopan Health Center, it is recommended that the Puskesmas improve cross-sectoral and cross-program coordination better. Efforts to increase public awareness about TB are also very important. In addition, training for health workers regarding treatment supervision techniques and how to support patients can help increase the recovery rate of pulmonary TB.

Conclusion

The management of the Lung TB Management Program with a DOTS strategy at the Kotanopan Health Center has shown some challenges, including a low healing rate and fluctuations in the number of cases. However, with a more comprehensive and integrated approach, it is expected that the program can achieve the targets set and have a positive impact on public health.
