Introduction

Early breastfeeding initiation (IMD) is a crucial step in the success of exclusive breastfeeding and breastfeeding duration. It is essential for the health and growth and development of babies. However, the implementation of IMD in private practice midwives in Binjai City in 2015 was not optimal. This study aims to analyze the factors that influence the implementation of IMD in private practice midwives in Binjai City in 2015.

Methodology

This study uses an analytical observational approach with a cross-sectional design. The research population includes all private practice midwives in Binjai City, which is 340 people. The research sample amounted to 111 people selected using the proportion method. Data collection was carried out through questionnaires and observations to analyze the influence of midwife characteristics such as age, education, working period, training, knowledge, attitudes, and motivation to the implementation of IMD.

Results

The results showed that the training of private practice midwives had a significant influence on the implementation of IMD. Midwives who have participated in training have a greater opportunity to carry out IMD. The training variable is the dominant factor that affects the implementation of IMD. In addition, the knowledge of private practice midwives is also proven to affect the implementation of IMD. Midwives who have good knowledge about IMD have a higher chance of implementing IMD.

Discussion

The findings of this study are consistent with previous studies that have shown the importance of training and knowledge in the implementation of IMD. The training of midwives is essential to increase their knowledge and skills in implementing IMD. The knowledge of midwives is also crucial in providing education and support to mothers to start breastfeeding immediately after delivery.

The role of midwives in supporting the success of IMD cannot be denied. Midwives as health workers who are in direct contact with mothers and babies have a vital role in providing education and support for mothers to start breastfeeding immediately after delivery. This study shows that increasing the knowledge and skills of midwives through training is an important step to increase IMD numbers in Binjai City.

Conclusion

The implementation of IMD in private practice midwives in Binjai City in 2015 was influenced by several factors, including training and knowledge. The training of midwives is essential to increase their knowledge and skills in implementing IMD. The knowledge of midwives is also crucial in providing education and support to mothers to start breastfeeding immediately after delivery.

Recommendations

Based on the findings of this study, several recommendations are made. First, the Binjai City Health Office needs to increase training related to the implementation of IMD to midwives. Second, it is necessary to make a written policy about the promotion of IMD and the granting of rewards/rewards for midwives who carry out IMD in every assistance of labor. Third, the Indonesian Midwife Association (IBI) organization is expected to facilitate seminar and training activities on early breastfeeding initiation to increase the knowledge, ability, and skills of midwives in the implementation of IMD. Finally, it is expected that private practice midwives can increase awareness of the importance of IMD implementation in every assistance of labor, so that it can affect the process of breastfeeding longer until the baby is 2 years old.

Implications

The findings of this study have several implications for the implementation of IMD in private practice midwives in Binjai City. First, the training of midwives is essential to increase their knowledge and skills in implementing IMD. Second, the knowledge of midwives is crucial in providing education and support to mothers to start breastfeeding immediately after delivery. Third, the implementation of IMD requires a written policy and the granting of rewards/rewards for midwives who carry out IMD in every assistance of labor.

Limitations

This study has several limitations. First, the study only analyzed the factors that influence the implementation of IMD in private practice midwives in Binjai City in 2015. Second, the study did not analyze the factors that influence the implementation of IMD in other settings, such as hospitals. Third, the study did not analyze the long-term effects of IMD on the health and growth and development of babies.
